Get Involved at Congress

A physician must complete the sports physical. Here is the form you will need for the physical.  Students WILL NOT be able to participate without a current physical. Please note that this is a new form, old forms should no longer be used. This physical will need to be uploaded onto PRIVIT once completed. We are no longer accepting paper copies of physicals.

Sway is a platform that our athletic trainers can use for baseline testing, sideline assessments, and the return-to-play process through an ever-growing battery of balance, cognitive, and functional assessments. This will take place of ImPACT testing.

Our Athletic Department is changing our concussion testing for student athletes, effective immediately we will be moving from ImPACT to SWAY.

This will be scheduled for ALL student athletes even if they have a current ImPACT test on file and will be scheduled with the coaches and athletic trainers. 

There is required documentation and pre-work that must be completed before your athlete will be able to participate in the SWAY testing. Please review and complete the documentation below so that your athlete is prepared prior to testing day.

This coming school year, the Park Hill School District will be transitioning to Privit, a trusted platform for managing student-athlete health information.

With the implementation of Privit, parents and guardians will have convenient access to securely update and manage their child's health information online. This includes medical history, emergency contacts, physicals, SWAY concussion testing permission forms, and other required MSHSAA documents. By utilizing Privit we can ensure that our sports training staff and coaches have access to accurate and up-to-date medical information when needed.

To facilitate this transition, we are asking that all students who plan to participate in a MSHSAA-sponsored activity have a Privit account.

·         All students participating in a MSHSAA activity must create an account and complete the required forms. Click Here to Create a Privit Account. Detailed instructions and help with account registration can be found here.

·         Coaches and school staff will no longer collect paper copies of physicals. Instead, parents/guardians must create a Privit account and upload a picture or file of page 5 of the MSHSAA physical form. Click Here to Download the MSHSAA Physical Form.

·         Many students have a physical on file that is valid for two years. If your student currently has a valid physical on file with the Park Hill School District, you must still create a Privit account and complete the required forms. You will have the option to indicate that your student has a valid physical on file; school staff will then verify and upload the physical we have on file.

We believe that this transition to Privit will significantly improve the way we manage student health records. Thank you for your cooperation and understanding as we work to implement this positive change.
